The Bridge Behavioral Health has been looking for you As an Outpatient Counselor (LADC/LMHP) at The Bridge you will provide professional level counseling for the treatment and rehabilitation of clients referred for substance use/mental health. The Outpatient Counselor (LMHP/LADC) is a full-time position responsible for an average of 24-26 direct service hours per week including substance use evaluations, outpatient therapy, and psychoeducational and therapy groups.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES OF THE OUTPATIENT COUNSELOR (LMHP/LADC) AT THE BRIDGE INCLUDE:

  • Help The Bridge foster and maintain a welcoming, trauma-informed, and safe environment for all clients.
  • Conduct substance use evaluations using standardized assessment tools to determine client’s current needs and makes recommendations to appropriate level of care.
  • Lead psychoeducational and therapy groups as needed.
  • Work with staff to schedule initial and ongoing Outpatient counseling/therapy appointments.
  • Provides individual, group, and family counseling/therapy specific to the needs of the client.
  • Responsible for the effective development and regular update of treatment plans (meeting specific program guidelines) utilizing and documenting client preferences.
    • Use standardized assessment tools to assess client’s initial and ongoing situation including medical, employment, support, substance use history, legal, family history, family and social relationships, trauma, psychiatric symptoms and mental health concerns. Utilize this information and work with the client to update the individualized treatment plan.
    • Utilize trauma informed assessments and other tools to develop treatment plan and reviews plan with client regularly.
  • Responsible for the effective development and regular update/recording of counselor/therapy progress notes, discharge summaries and aftercare plans.
  • Consistently utilizes concurrent documentation as indicated by client signature on clinical documentation.
  • Conduct and coordinate clinical case-management, including ongoing contact and coordination with probation, parole, diversion officers, case workers, therapists/counselors, family members, and/or any outside source involved with the client, in a manner that aligns with the clients’ individual treatment goals and objectives.
  • Provide crisis intervention, as necessary, and the ongoing assessment of each client’s stability and functioning following a crisis event.
  • Complete timely documentation in the agency’s electronic health record (Credible).
  • Prepare and maintain all required treatment records and reports daily to ensure all clinical files are current and meet accreditation and various payer regulations and guidelines as well as meet best practice guidelines.
    • Complete all required documentation in a timely and professional manner, meeting the requirements of accreditation and varying payer sources (24 hours for notes and 14 days for assessments, evaluations, and discharge summaries)
  • Make appropriate referrals throughout the client’s treatment based on client need.
  • Participate in clinical team staffing as indicated by caseload/licensure. Document the staffing decisions and implement the appropriate treatment.
  •  Attend and participate in monthly team meetings and quarterly all staff training as directed.
  • Participate in regular supervision meetings with Director of Clinical Services.
  • Complete all monthly and annual online e-learning courses on time, in compliance with state and accrediting organizations training requirements.
  • Participate in training activities and credentialing through the Nebraska Department of Health and Human Services Regulation, Licensure and Credentialing Division, as well as the Nebraska Office of Justice Services.
  • Other duties as assigned.

 

MINIMUM REQUIREMENTS:

  • Licensure as Licensed Mental Health Practitioner (LMHP/PLMHP) - Preferred 
  • Licensure as Licensed Alcohol and Drug Counselor (LADC/PLADC) 
  • Must be eligible to work in the United States
  • Must be 21 year of age or older. 


PREFERRED: 

  • Licensed Mental Health Practitioner (LMHP/PLMHP)
  • ASI/CASI training and on Preferred Provider list
  • Experience working one on one with clients (case-management, counseling, therapy). 

 

KNOWLEDGE, SKILLS, ABILITIES:

  • Two years of experience working with people with substance use histories.
  • Ability to work within a team concept, establish and maintain professional boundaries with clients, and coordinate with other community agencies.
  • Ability to manage multiple tasks in a timely manner
  • Ability to utilize 12-step programming and other treatment modalities effectively
  • Ability to look at problems analytically and solve them creatively.
  • Organizational skills and ability to prioritize assignments.
  • Superior verbal, written, and presentation skills.
  • Ability to maintain confidential information.
  • Ability to work with personnel with all levels of expertise.
  • Must have the ability to learn and become proficient in a variety of electronic and web-based platforms to include Credible and Microsoft Office
  • Must have the ability to work with persons of multiple ethnic, racial, cultural and varied economic backgrounds.
