Responsibilities

  • Collaborate with clients to identify strengths, challenges, and resources to support their mental health journey.
  • Provide ongoing support and encouragement to clients, fostering a safe and non-judgmental environment.
  • Connect clients with appropriate community resources and services to enhance their treatment outcomes.
  • Coordinate and communicate with other professionals, such as therapists, doctors, and social workers, to ensure holistic care for clients.
  • Regularly monitor progress and reassess treatment plans to ensure the effectiveness of interventions.
  • Advocate for clients and empower them to actively participate in their recovery process.
  • Conduct home visits or accompany clients to appointments when necessary.
  • Maintain accurate and up-to-date documentation of all client interactions and interventions.
  • Participate in case conferences, team meetings, and professional development activities.

Qualifications

  • A bachelor's or master's degree in social work, psychology, counseling, or a related field.
  • Possess strong interpersonal and communication skills to collaborate effectively with clients and team members.
  • Experience working in a mental health or social services setting is preferred.
  • Knowledge of community resources and the ability to facilitate client access to these resources.
  • Ability to maintain confidentiality and demonstrate ethical behavior in all situations.
  • Excellent organizational and time management skills to ensure effective case management.
  • Proficient computer skills for documentation, data entry, and communication purposes.
