What you can expect:

Reporting to the Children Supervisor, our Children’s Case Manager provide a range of individual, family, and group counseling services to assigned individuals from a range of diagnostic categories and needs (mental illness, severe emotional disturbance, co-occurring mild intellectual/developmental disabilities and co-occurring disorders).

How you will make an impact:

  • Coordinates person-centered planning process for assigned consumers and families including arranging for meeting times, invitations, and completion of the final plan and accompanying documents (i.e. individual and family crisis plans) per MDHHS guidelines.
  • Provides face to face recovery focused treatment, as outlined in person-centered plan, utilizing core competencies of Case management: assessment, planning, linking, coordinating, and monitoring.
  • Provide services using evidence based or promising practice treatment modality.
  • Documentation completed is timely, strengths based, utilizes recovery methods and measures consumer progress and satisfaction with services.
  • Complete level of care assessments at designated intervals (intake, every 90 days, annual and at time of discharge).
  • Issue action notices to families after attempts at re-engagement and no activity for 30 to 45 days, completing a discharge summary and/or transfer document when effective date arrives without appeal. Reassess the level of care if 90 days of no activity occurs.
  • Act as liaison to family treatment court and juvenile probation, schools, Intermediate School District, MDHHS children’s protective services and foster care, MDHHS contracted foster care agencies, pediatricians, and/or residential facilities to promote appropriate treatment and to coordinate care and discharge planning.
  • Coordinate care with other external organizations as necessary (DHS, courts, law enforcement, social security, transportation, community resources, medical providers, etc.).
  • Provide or arrange for crisis intervention services when needed.
  • Coordinate care with primary care physician/pediatricians and Medicaid Qualified Health Plans

Minimum Education & Experience Requirements:

  • BA/BS/MS/MA
  • Eligible for Michigan Social Work Licensure or QMHP/QIDP/CMDP

ADA Specifications:

  • Work is normally performed in an office setting
  • May be required to sit for long periods of time
