Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
  • Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
  • Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
  • Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Job Title: Nurse Manager

Reports to: Clinical Health Services Director

Areas of Responsibility: Plans, directs, and coordinates nursing and ancillary departments in order to provide safe and quality care to patients. Supervises Nursing Supervisors, Immunization, Nutrition and Referrals departments across a multidisciplinary clinic increasing effective communication and teamwork. Manages compliance tasks including policy and procedure development along with safety initiatives.

Major Duties:

  1. Promote, oversee, and play an active role in delivering value-based patient care using the provider-led, team-based Patient Centered Medical Home Care Model.
  2. Collaborate with Providers and Nursing Supervisors in the provision of care and services that are “accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate, and culturally effective.”
  3. Ensure continual delivery of high-quality nursing care while adopting and implementing innovative nursing practices to improve our facility and patient care.
  4. Advocate for an integrated health care system, working with nursing, MSS, nutrition services, ancillary staff and providers to ensure quality, safe outcomes in patient care and promoting referrals within the agency.
  5. Develop a strategic vision for the nursing and ancillary staff including onboarding initiatives, clinical competencies and continuing education activities which promotes staff engagement, professional development, and performance improvement measures while meeting regulatory standards.
  6. Facilitates coordination of nursing services between departments. Maintain a collaborative relationship between clinical staff by communicating information, responding to requests, building rapport, participating in team problem-solving methods.
  7. Monitor and assist in developing recommendations for: internal referrals, external referrals, warm handoffs, missed appointments, and patient tracking systems.
  8. Assures quality of care by developing and interpreting clinic and nursing department’s philosophies and standards of care, enforcing adherence to State Board of Nursing and State Nurse Practice Act requirements and to other governing agency regulations, measuring health outcomes against standards, and making or recommending adjustments.
    • Maintains safe and clean working environment by designing and implementing procedures, rules, and regulations, and by calling for assistance from other health care professionals.
    • Protects patients and employees by developing and interpreting infection-control policies and protocols, and by enforcing medication administration, storage procedures, and controlled substance regulations.
    • Provides input on annual budget and financial reporting, monitors expenditures, performs research on possible investments in equipment or additional staffing, and identifies variances in departmental costs.
  1. Provide oversight of clinical quality assurance compliance. Uses adopted models to appropriately measure metrics and processes for quality assurance. Works with the Managed Care Organizations on their quality performance measures and incentive programs, ensuring guidelines are met.
  2. In coordination with Quality Improvement Coordinator, assist with the management, documentation, and submission of screening data, as well as other quality assurance/improvement measures as required by grants and clinic strategic plan and QA Committee.
  3. Oversee the administration and implementation of Immunization Program.
  4. Develop, implement and manage clinical policies and procedures with appropriate stakeholders ensuring compliance with state, federal and grant requirements. Ensure that staff maintain these standards and policies.
  5. Performs supervisory administrative tasks including approving time off requests and timecards, setting goals, conducting annual reviews and developing performance improvement plans or corrective action as needed, conducting candidate interviews and defining successful onboarding processes.
  6. Maintain a healthy, collaborative team environment, striving to openly communicate with staff, peers, and patients, breaking down communication barriers when necessary.
  7. Receive and address patient complaints following appropriate grievance procedures.
  8. Promotes good public relations with patients, visitors, and the community.
  9. Assist in maintaining a safe environment for patients and their families.
  10. Facilitates and coordinates meetings as needed.
  11. Maintain confidentiality of records and communications following HIPAA privacy and security policies and procedures.

Working Conditions:

  • Office environment for training and meetings in addition to standard clinic environment.
  • Interaction with members of the community and a variety of outreach opportunities.
  • Well-lit, temperature-controlled clinical setting, with standard medical and office equipment available.
  • Work may require meeting tight deadlines and ability to successfully multitask.
  • May be required to work irregular hours and attend out of town training.
  • Available for irregular working hours as needed.

Physical Requirements:

  • Ability to frequently bend, stoop, squat, walk and pull drawers.
  • Manual dexterity sufficient to operate office and medical equipment.
  • Must regularly talk, hear, and have close vision ability to record and communicate appropriate documents and records.
  • Physically and mentally able to perform duties.
  • Occasionally lifting and carrying up to 20 pounds.
  • Ability to push a patient in a standard manual wheelchair.

Minimum Qualifications:

Grade: 11

  1. Licensed Registered Nurse with a Bachelor’s Degree as an RN, preferred.
  2. Minimum of four years’ experience in general nursing and two years of management experience.
  3. Current CPR certification.
  4. Ability to work efficiently under time constraints.
  5. Age 21 or above with a valid Nebraska driver’s license and insured vehicle.
  6. Skilled facilitator and problem solver with excellent communication and interpersonal skills.
  7. Detail oriented person with demonstrated organizational skills and the ability to complete projects on time with minimal supervision.
  8. Strong leadership skills and ability to work collaboratively with a large team.
  9. Possess solid business sense and strong communication skills.
  10. Must have ability to work independently and to exercise initiative, good judgment, and decision-making skills.
  11. Must be able to operate computer and most office software and equipment.

Grade: 12

  1. Must meet the minimum qualifications above;
  2. Minimum of six years’ experience in general nursing and four years of management experience.

Grade: 13

  1. Must meet qualifications above;
  2. Minimum of eight years’ experience in general nursing and six or more years of management experience.
  3. Specialized training in related field.
