As a Home Care Coordinator, you will play a pivotal role in coordinating and managing the care services provided to our clients. You will be responsible for assessing client needs, developing personalized care plans, and scheduling caregivers to ensure the highest quality of care. Additionally, you will support the recruitment and onboarding process for new caregivers, maintain client records, and provide ongoing communication with clients and their families.
You will:
- Conduct initial and ongoing assessments of clients' needs to develop personalized care plans
- Manage and maintain client records, ensuring accuracy and confidentiality
- Conduct regular check-ins with clients and caregivers to ensure quality of care
- Collaborate with the care management team to address any client concerns or issues
- Recruit, train, and supervise home care aides, providing support and guidance to ensure high-quality care.
- Regularly visit clients in their homes to assess the quality of care, provide support, and ensure that care plans are being followed.
You will have:
- At least 2 years of experience in home care coordination
- Licensed Nursing Assistant (LNA) certification
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work collaboratively with a diverse team
- Proficiency in electronic health record (EHR) systems
