Case Manager

- The Case Manager provides case management services to children and families in order to facilitate safety, permanency, and well-being.
Location:
- Olathe, KS
Sign on Bonus:
- A $2,000 sign on bonus is associated with this job.
Education:
- A bachelor’s or master’s degree in human services related field.
Experience:
- Previous experience with children and family services preferred.
Licensure/Certification:
- Valid drivers license
- Valid Auto Insurance
- At least 21 years old
- Licensure by the Kansas BSRB is preferred.
Essential Duties:
- Actively seek to understand and support families’ lived experience and individual needs.
- Needs include, but are not limited to, developmental stages, trauma, poverty, gender, race, ethnicity and language.
- Build and maintain strong engagement with clients, placement providers, DCF, coworkers and other community partners.
- Actively share relevant and appropriate information with family and involved professionals.
- Provide timely responses to all requests.
- Provide home and community-based services to clients.
- Services include, but are not limited to, observation of caregiver/child interaction, maintain both existing and developing connections, resource coordination, on-call crisis intervention, safety planning, skill building, transport clients and conduct drug-screens to ensure safety and well-being.
- Utilize information gathered through assessment process to advocate for the client’s best interest.
- Advocacy includes, but is not limited to, provide court testimony, attend school meetings and coordinate mental health services
- Complete required documentation in a timely and thorough manner.
- Documentation includes, but is not limited to, case logs, comprehensive assessments, court reports and social histories.
