Thome PACE® serves Jackson, Lenawee, and Hillsdale counties providing participants a wide range of medical and supportive services in our Day Clinic located in Jackson, Michigan or in the comfort of their own homes. The Thome PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve.

Summary:

The Registered Dietitian is independent in the application of advanced dietitian knowledge and skills and is an important member of the Interdisciplinary Team (IDT). Responsibilities include: evaluate the participant's nutritional status, develop and implement nutritional care plans, assess nutrient requirements, recommend nutritional prescriptions, evaluate participant tolerance and effectiveness of nutritional care, and communicate nutritional data and information to the physicians and other health professionals. The Registered Dietitian works under the supervision of the Lead Registered Dietitian.

As we age, we often require a helping hand and medical assistance. Thome PACE (Program of All-Inclusive Care for the Elderly) understands this need and provides a welcomed alternative to nursing home care for participants. The PACE model of care allows seniors aged 55+ to remain independent in their homes and communities while receiving the integrated social, wellness, and medical care services aging adults require.

Position Functions and Duties:

  • Performs nutritional assessments and reassessments as indicated by nutrition standards.
  • Identifies need and implements appropriate nutrition services, which may include home delivered meals and nutritional supplements.
  • Provides nutrition education to participants/caregivers as identified independently or ordered by providers.
  • Participates as a member of the Interdisciplinary Team. Attends meetings and takes part in participant care planning.
  • Works collaboratively with other IDT members.
  • Documents participant changes appropriately in the medical record and communicates participant changes to team members.
  • Assists in monitoring meals including intake, diet compliance, and tray accuracy.
  • Coordination of 24-hour care delivery

Required Qualifications:

  • Bachelor’s degree or master’s Degree from an ACEND accredited program and completed the Dietetic Internship.
  • Passed the CDR exam and maintain Registration status in good standing. Those who are eligible to sit for the CDR exam may be considered.
  • Must be CPR certified or willing to obtain CPR within 90 days if offered employment.

Desired Qualifications:

  • Master's Degree plus approved work experience preferred.
  • Certified Diabetes Educator (CDE) status preferred.
  • Minimum of one (1) year working with the frail and elderly.
