Job DescriptionJob Description

The Hospice RN Case Manager is responsible for collaborating with the interdisciplinary team, attending physician and/or PCP to manage client’s care. The Hospice RN Case Manager will be in charge of assessment, evaluation, interventions, coordination of care based on the client’s individual needs. They will educate the client and family on symptom management, disease process, and hospice philosophy. The RN Case Manager oversees the Hospice Aide’s work. This position reports to the Hospice Clinical Director.

There is a full time and per diem positions available.

1. Provides comprehensive assessment, planning, and implementation for a caseload of hospice clients as the RN Case Manager. Uses health assessment data to determine nursing diagnoses and provides care requiring specialized RN skills. Synthesizes information into a comprehensive plan of care to meet the individual client’s needs. Makes necessary recommendation for level of care changes and care plans changes when the client’s health status and needs change. Updates the Interdisciplinary Team (IDT) with changes and recommendations.

2. Provides education to the client and client supports as needed regarding the plan of care. Communicates with community resources as needed to coordinate the plan of care.

3. Assumes responsibility for coordination of medical orders and MD communication regarding the client’s needs and reports any changes in condition. Obtains and receives MD orders. Communicates and collaborates timely and professionally with other members of the IDT.

4. Participates in on-call duties as assigned, including telephonic triage. Be available to provide after-hours services as assigned, including admissions. The RN Case Manager ensures equipment and supplies needed are provided timely to the client.

5. Participates in continuing education programs and in services, and case conferences with other IDT members as assigned. Assists in the supervision and evaluation of care provided by Hospice Aides as required by regulatory standards. Provides education as needed to ensure the Hospice client is receiving safe and effective care. Appropriately delegates per the Wisconsin Nurse Practice Act.

6. Exhibits sound nursing judgment and decision-making skills in coordinating client care.

7. Provides direction and leadership within the Hospice Department regarding nursing practice to ensure care and quality standards are met, compliance with regulatory requirements is in place and met. Assists with staff education regarding Oakwood policies and protocols.

8. Fosters professional development of self and others in the department.

9. Maintains productivity according to agency standards after completing orientation. Uses technology while maintaining HIPAA and client confidentiality. Uses good judgment regarding fiscal responsibility, time management, and cost control activities.

10. Demonstrates flexibility with scheduling within a face-paced and changing environment while maintaining a positive attitude.

11. Maintains professional boundaries with clients, ensuring professional, client-centered care.

12. Supports others in the department through group decision making, resolving conflict, and managing stress. Serves as a preceptor to new staff, as assigned.

ESSSENTIAL QUALIFICATIONS

1. Knowledge and ability to apply current nursing theory and practices with an emphasis on assessment skills and problem-solving techniques as they relate to resident needs.

2. Motivates residents to participate in their care team coordination.

3. Learns and implements state and federal regulations that affect the care of Hospice clients.

4. Fulfills job responsibilities in a professional, accurate, and conscientious manner.

5. Creates and maintains an atmosphere that fosters effective team relationships with other staff and with various community agencies and with the public.

6. Manages confidential information according to HIPAA guidelines.

7. Exercises independent judgment and make sound decisions.

8. Provides on-call support as assigned, including phone triage and after-hours admissions.

9. Maintains a flexible work schedule to meet the needs of clients and staff.

10. Knowledgeable about hospice Conditions of Participation and Standards of Care.

11. Works within departmental and organizational policies and procedures.

12. Participates in the monitoring of nursing skills of peers and Hospice Aides using quality assurance tools as appropriate.

EDUCATION, EXPERIENCE AND/OR TRAINING

1. Associate Degree of Nursing (Bachelor’s degree preferred)

2. Current Unrestricted Wisconsin RN license

3. Hospice experience required. Prefer three or more years of nursing experience.

4. CPR certification.

5. Valid Wisconsin driver license and car insurance.
