We envision being an essential member of a community network that helps prevent behavioral health disorders and provides the care and support people need to lead healthy, productive lives.
The Health Home Case Manager will be a liaison with the client, family members, healthcare providers, and insurance companies. Some of the tasks include;
- Advocating and coordinating for the psychiatric and somatic care of our clients. Work with pharmacies to obtain medications, supplies, and durable medical equipment.
- Coordinate care with physicians, hospitals, pharmacies, nursing facilities.
- Assist with finding providers and specialists that accept client's MCO or Medicare.
- Coordination of residential hospital admissions and discharges.
- Scheduling labs and toxicology screening.
- Assisting prescribers and RN with obtaining pre-authorizations.
- Assist with residential hospital admissions and discharges.
Education; BS/BA/AA in Health, Human Services, or Certified Nursing Assistant.
Experience; One year experience working in Human Services is preferred, a Certified Nursing Assistant is a plus.
