Elder Justice Advocate

(Victim Service Case Manager)

for Saint Elizabeth Community's Haven for Elder Justice Program

Full Time | Day Shift

The safety of elder victims of abuse is the primary goal of the Haven and the work of the Elder Advocate. The Elder Justice Advocate provides advocacy, support, and coordinated services to meet the needs of elder victims of abuse in our community-based program and shelter program. The Elder Justice Advocate is responsible for providing outreach and community engagement activities in diverse communities and victim-service case management to elder victims. They will work to identify and engage elders and service providers in underserved communities to build working relationships and create a network of resources.

Responsibilities of an Elder Justice Advocate include:

  • Provide shelter and community-based victim services.
  • Assess an elder's need for shelter, admission eligibility and work with the facility to transition the elder into shelter.
  • Assess an elder's need for community-based services.
  • Develop and implement a safety and care plan (including discharge plan for sheltered elders) addressing basic, legal, health and mental health needs.
  • Coordinate services and resources with state and community agencies to meet the elder's needs.
  • Convene Community Resource Teams as needed to address the care, services, and safety planning for elder victims.
  • Accompany law enforcement on safety check visits upon request.
  • Accompany elder victims to the ER upon request.
  • Provide elder abuse education and safety planning for all elders.
  • Advocate for legal services and assist with issues of Temporary Protective Orders, Victim's Compensation, victim impact statements, and accompany elder victims through the court and prosecution process.
  • Provide follow up services to elder victims once discharged from the program.
  • Maintain detailed case records and provide reports.

Qualifications for an Elder Justice Advocate:

  • Bachelor Degree, required.
  • Bachelors Degree in Social Work, Nursing or related field preferred.
  • 1-2 years working with elders.
  • Prior case management experience preferred.
  • Possession of a valid driver's license and a registered, insured vehicle.
