Job DescriptionJob DescriptionDescription:

The RN Case Manager is responsible for the coordination, referral, and support of a conflict-free case management caseload utilizing a person-centered approach. Additional responsibilities include outreach/marketing activities as needed and appropriate, and responding to requests for information and/or assistance from individuals in a timely manner.


Case Managers will be required to provide most services on-site at group homes or other community-based locations but will have the ability to provide some services remotely from home if a private workspace is available in order to ensure patient privacy.

Requirements:

Education/Experience:

  • Active RN license in the state of RI
  • Driver's License and reliable transportation
  • Experience working with adult IDD (Intellectual and Developmental Disabilities) population
  • Experience working with individuals with chronic illness and behavioral health issues
  • Experience completing comprehensive assessments and associated individualized care plans
  • Experience navigating community and state resources within Rhode Island
  • Experience in person-centered care planning and working with individuals to establish and pursue goals
  • Conflict-Free Case Management and/or Person-Centered Care training preferred

Job Skills:

  • Ability to work as part of an interdisciplinary team and actively participate in team meetings and case conferences
  • Excellent communication skills and the ability to interact respectfully with culturally and linguistically diverse populations
  • Ability to work independently
  • Ability to communicate effectively regarding self-direction (risks and responsibilities)
  • Competent in demonstrating the core components of case management (information gathering, person-centered planning, connecting to services and supports, and plan monitoring with follow-up
  • Capable of providing case management services in an efficient, effective, and collaborative manner to help avoid duplication of services, costs and administrative tasks.
  • Respectful of individual's rights, strengths, values and preferences; encouraging the individuals to create, direct and participate in their person-centered plan to the fullest extent possible.
