SMART Integrated Behavioral Health Clinician

SMART Purpose: Our purpose is to break the cycle of poverty and positively impact the trajectory of lives. We achieve this by identifying the physical, social, emotional, and behavioral barriers to classroom success and then delivering interventions to address those barriers, and thus creating optimal conditions for learning through the deployment of Active Access to deliver Active Care for ALL students, school faculty and staff, and students’ families. Integrated Behavioral Health Clinicians ensuring the team’s ability to view students’ and their family’s care needs through a “Whole Child, Whole School” lens, treating social, emotional or behavioral issues created by underlying medical conditions, lifestyle factors or resulting medical condition from those factors.
LCSW/Population Care Coordinator Position Summary: Manage the care of the patient population by initiating and administering, Primary, Secondary, and Tertiary tiered interventions, commencing with an integrative health assessment, which assists with risk and problem identification, then identifying and implementing appropriate interventions and follow-up strategies. Working in collaboration with the FNP, the SMART team, and the student, and in some cases school personnel, define expected patient outcomes, describe appropriate resource utilization, and develop individual goal plans. Provide support/care coordination to the practice program, in order to optimize patient health and academic outcomes, while maximizing existing resources/partnerships within the school and minimizing duplication of services. Emphasize supporting academic success in each plan of care and goal. Utilize aggregate population data to ascertain major population risk factors and/or social determinants and work collaboratively with the school partner and SMART team to identify and prioritize risks and create and execute actionable solutions to mitigate the negative impact of those social determinants. Work to Maximize SMART’s capacity to support a trauma-informed framework and healing, in keeping with the SMART Whole Child, Whole School approach.
