DELVE DEEPER INTO THIS NURSING ROLE

As our LPN - Licensed Nurse, you'll be the crucial link between behavioral health and primary care, ensuring seamless collaboration for our patients. Your responsibilities will include initiating, updating, and monitoring comprehensive person-centered care plans, supporting schedules, reducing barriers to medical and behavioral health appointments, and providing high-touch, in-person support to ensure treatment adherence. You'll also engage in proactive outreach, liaise with external providers, and identify community resources. Your role will be instrumental in developing discharge plans, promoting integrated care, and administering prescribed medical treatments.
