YHDP Case Manager

Job Purpose
The Youth Homeless Demonstration Project (YHDP) Case Manager helps youth and youth adults (17-24 years of age) find affordable, safe, quality, and accessible housing. They work with their communities to encourage the expansion of housing options.
Qualifications
- Minimum of high school diploma or equivalent.
- Demonstrates ability through prior work experience to be competent in the position.
- Lived expertise in overcoming mental health challenges/homelessness.
Core Job Duties
- Explain policies and procedures of the YHDP Subsidy Housing Program.
- Interpret qualifying information from person(s)-served and determine eligibility.
- Perform technical and administrative duties in support of Housing Programs.
- Monitor housing properties for compliance with State requirements and federal regulations.
- Recommend and assist in the implementation of goals and objectives.
- Establish schedules and methods for providing affordable housing services, while utilizing policy and procedures.
- Demonstrate a working knowledge of diagnostic criteria set for substance use and/or mental disorders.
- Adherence to all applicable evidence-based practice models.
- Orally communicate information effectively and accurately.
- Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
- Abide by program and agency policies and procedures.
- Other job duties and special projects as assigned.
