Job Summary:

Our client is an innovative, rapidly growing medical group that is changing the way primary care is delivered to seniors. Our state-of-the-art centers are in communities where there is little to no quality healthcare today. The healthcare model is entirely driven by providing better primary care to our patients, not by the volume of services we provide. Ideal candidates will be:

  • Currently licensed to practice professional nursing or Physician Assistant in the state of employment, active and non-probationary
  • Experienced in primary care: internal medicine, geriatrics, or family medicine
  • Current BLS/ACLS certification
  • Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions
  • Strong understanding of examination methodologies and diagnostics
  • Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members
  • Confident in their ability to deliver high quality primary care, which includes being available to patients and addressing their medical and social needs
  • Passionate about their work and energized by being part of a fast growing organization
  • Comfortable traveling within the market to multiple centers within short distance of each other

* Maintain complete and detailed records of patients' health care plans and prognoses.
* Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability.
* Prescribe medication dosages, routes, and frequencies, based on such patient characteristics as age and gender.
* Counsel patients about drug regimens and possible side effects or interactions with other substances, such as food supplements, over-the-counter (OTC) medications, or herbal remedies.
* Advocate for accessible health care that minimizes environmental health risks.
