SUMMARY OVERVIEW
The Nurse Manager plays a pivotal role in overseeing the Nursing program at Boston Senior Home Care. Key responsibilities include supervising and coordinating BSHC Care Nurses to ensure compliance with program goals, participating in interdisciplinary teams for client risk assessments and service planning, and conducting on-site assessments to address consumer needs.
The role involves ensuring medical and clinical care needs are integrated into care management plans, leading troubleshooting efforts to resolve issues between staff, and developing processes for identifying consumers for special programs. The Nurse Manager also participates in Quality Assurance activities, acts as a clinical decision-maker in case management reviews, and provides consultation and feedback on client care plans.
Additionally, the Nurse Manager is responsible for developing and implementing training programs, setting goals and expectations for staff, providing team building and motivation, and collaborating in the performance evaluation process of staff. The role requires continuous development and oversight to maintain high standards of care and compliance with regulatory requirements.
ESSENTIAL FUNCTIONS

- Supervision and Coordination: Supervises, directs, and coordinates the activities Nurses, reviewing caseloads, disseminating workload activities, and providing guidance to ensure program goals and compliance are met.
- Interdisciplinary Team Participation: Functions as part of the interdisciplinary team with the Program Manager, Direct Care Program, Staff Supervisors, and Nurses, participating in risk assessment and service planning coordination for clients.
- On-Site Assessments: Conducts on-site assessments to evaluate the mental, physical, and medical needs of consumers, performing risk assessments, and ensuring that these needs are addressed in the consumer care plan.
- Care Management: Ensures medical and clinical care needs are included in care management plans and leads the troubleshooting of issues to achieve client care plan objectives, collaborating with staff to promote cost-effective service plans.
- Special Programs Management: Leads the development and oversight of processes for identifying consumers for special programs (e.g., CHOICES, CTLP, AFC), assisting in report creation, reviewing consumer information, and ensuring timely referrals.
- Quality Assurance: Participates in agency Quality Assurance activities, including external and internal audits.
- Clinical Decision Making: Acts as the lead clinical decision-maker in case management peer reviews, providing consultation and corrective feedback to nurses and case managers on various issues critical to consumer care plans.
- Training and Development: Develops and implements service training programs, offering technical expertise on mental health, behavioral, and medical issues, and coordinating in-service programs for agency staff.
- Staff Development: Sets goals and expectations for staff, providing team building, motivation, verbal and written guidance, managing discipline, and orienting and training new and existing staff.
- Performance Evaluation: Collaborates with the Nurse, Senior Director in the annual and ongoing evaluation process of Nursing staff.
- Additional Duties: Performs other duties as assigned.

This position supervises the Home Care Nurses.
WORK ENVIRONMENT
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, copiers, filing cabinets.

POSITION TYPE/EXPECTED HOURS OF WORK
This is a full-time position, Monday-Friday, 9:00 AM -5:00 PM
TRAVEL
This position requires up to 25% travel.
Must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions.
REQUIRED/PREFERRED EDUCATION AND EXPERIENCE
- Must have a Bachelor's degree in Nursing or Human Services and at least three years of experience, one of which must have been in a community setting and at least one year of supervisory, administrative, or managerial experience; or an Associate's Degree in Nursing and five years of experience with two years of community-based nursing or long-term care direct services experience.
- A valid Registered Nurse's license from the Commonwealth of Massachusetts and a Class 3 Motor Vehicle license.
- Knowledge of Psychiatric, Mental Health, Geriatric Nursing, disabilities and the aging process.
