We are seeking a dedicated and compassionate Nurse Practitioner to join our dynamic healthcare team. The ideal candidate will be responsible for providing exceptional patient care through assessments, medication management, and care coordination. You will serve as a crucial liaison between healthcare professionals, patients, and their families, ensuring seamless communication and continuity of care.

Overview:

The role involves conducting assessments, managing medications, coordinating care, and serving as a liaison among healthcare professionals, participants, and their families.

Responsibilities:

- Perform health histories, physical exams, and provide patient education.

- Oversee medication management and diagnostic tests, coordinating care under physician oversight.

- Act as an intermediary between healthcare professionals, participants, and families.

- Engage in provider schedules and participate in Interdisciplinary Team meetings.

- Aid in care coordination, document participant status changes, and educate on Advanced Directives.

Qualifications:

- Master's degree in clinical nursing from an accredited Nurse Practitioner program.

- 3-5 years of nursing experience, preferably in a Nurse Practitioner role.

- Capability to practice within scope, meet standardized competencies, with a preference for experience working with frail or elderly populations.

- Medically cleared for communicable diseases and current on immunizations.
