As a Residential Nurse, your task will be to provide the wellness services that are essential to helping our clients lead healthy and fulfilling lives, in both group home and community-based waiver settings. Your day-to-day responsibilities will go beyond the basics by providing healthcare coordination, working closely with the clients, their families, and healthcare providers to develop personalized plans for their unique needs and health goals. When not working directly with the clients, you’ll also be supporting the staff by teaching Core A/B medication administration classes, training them in wellness care plans, and providing quick consultation support for any unexpected client health needs.

Job Description 

POSITION TITLE: Nurse  - Wellness Coordinator

DIVISION:  Nursing                                

SUPERVISOR: Director of Nursing

CLASSIFICATION: Non-Exempt 

STATUS: Full- Time                               

PURPOSE:

The Nurse is responsible for providing health programming and medical services to persons served participating in the Waiver Wellness Coordination program.

KEY AREAS OF RESPONSIBILITY: 

  1. All service standards for the Wellness Coordination program are adhered by.
  2. Develop and oversee the maintenance and routine monitoring and training of Waiver participants Wellness Support plans per the Wellness Standard guidelines.
  3. Complete face to face assessments in accordance with the Wellness Tier Guidelines. Consults as determined by current healthcare needs.
  4. Complete Wellness Quarterly reports
  5. Follow treatment and medication plans for clients as recommended by a physician.
  6. Assist in the scheduling of medical appointments. Attend medical or specialty appointments when necessary.
  7. Maintain accurate medical files for clients. Prepare and maintain other records pertaining to medical information.
  8. Provide direct care and first aid to clients.
  9. Plan and present Infectious Disease Training for employees as necessary
  10. Maintain great communication and professional relationships with clients, family members and co-workers.
  11. Monitor, order, and stock medical supplies.
  12. Coordinate/ administer physician ordered injections/inoculations as needed.
  13. Review of Medication Administration record/Physicians' orders
  14. Coordinate/conduct medical in-services as needed.
  15. Must meet physical requirements. Physical requirements may include but not limited to bending, reaching, stretching, lifting 25-50 pounds, running, walking, squatting, assistance with physical transfer, handling physical aggression as defined in CPI training.
  16. Teach agency Med Core A & B classes as needed.

QUALIFICATIONS:

  1. Must be at least 18 years of age.
  2. Licensed Registered Nurse of Licensed Practical Nurse
  3. Maintain CPR, TB, First Aid and CPI certifications
  4. Driver’s license (Required)
  5. Possess and maintain a clear criminal record.
  6. Good Communicator, Dependable, and Compassionate
