They lead a team of RN Case Managers, Clinic RNs, Medical Assistants, and other staff to deliver high-quality care to participants. Key responsibilities include data analysis for process improvement, compliance with care plans, and collaboration with medical practice leadership.
Essential Job Duties:- Operate and maintain a high-performing, safe clinic with efficient workflows and clear protocols.
- Recruit, mentor, and manage RN Case Managers, Clinic RNs, Medical Assistants, and other team members to foster engagement and optimal performance.
- Analyze data to identify trends and opportunities for improvement, making recommendations to enhance clinic operations.
- Ensure accurate documentation and delivery of interventions as per participants' care plans, collaborating closely with medical providers and interdisciplinary team (IDT).
- Lead clinical and nursing audits in coordination with Quality Improvement Director to ensure compliance and develop corrective action plans.
- Bachelor’s degree in nursing or healthcare field preferred; equivalent professional experience considered.
- Current, unencumbered RN state license required.
- Minimum of two (2) years overseeing operations in an ambulatory clinic or equivalent nurse leadership in a high-volume outpatient setting.
- Minimum of two (2) years of clinical experience with a frail or elderly population preferred.
