Peaceful Living is seeking a Program Specialist in our Residential Department. The primary function of the Program Specialist is to provide and promote for the welfare, well-being, the care and programmatic needs of residents that we support, according to the philosophy, mission and vision of Peaceful Living. The role of the QIDP is central to the overall responsiveness and effectiveness of an active treatment program for the 'Intermediate Care Facilities for Individuals with Intellectual Disabilities' (ICF/IID) program.

Essential Job Functions

  • Coordinates and complete assessment for each individual, as required by Department of Health regulation.
  • Complete IPPs for each individual in the home and facilitate team meetings as appropriate. Schedules the 90 day IPP review and facilitates the meeting.
  • Ensure that program documentation is completed in an accurate manner and includes data that measures criteria as developed in the IPP.
  • Routinely observes the individuals across all program areas to assess effectiveness of program implementation and to determine effectiveness of program planning as related to staff training. Facilitates timely modifications to programs so that desired skills and goals are achieved.
  • Interacts with staff across program area to determine the effectiveness and relevance of program plans to meet identified needs of the individuals.
  • Determines the need for program revision based on individuals' outcome performance.
  • Identifies inconsistencies in staff training approaches or programs not implemented as developed and facilitates the remediation of the inconsistencies.
  • Monitors and ensures that follow up for all recommended services, equipment, and programs to assure all needed services and supplies are provided in a timely manner to meet the needs of the individuals.
  • Coordinates internal and external program services and supports to facilitate the acquisition of skills.
  • Promotes competent interactions of Direct Support Professionals with each individual in program implementation and behavioral support management.
  • Coordinates with professional and non-professional staff to provide direct and indirect services to meet the needs of the individuals.
  • Participates in the Human Rights Committee and provides documentation and recommendations as appropriate.

Education/Experience

  • A master's degree or above from an accredited college or university and 1 year work experience working directly with individuals with an intellectual disability. A bachelor's degree from an accredited college or university and 2 years work experience working directly with individuals with an intellectual disability; associate's degree or 60 credit hours from an accredited college or university and 4 years work experience working directly with individuals with an intellectual disability.
