Professional Qualifications:

Nurse Liaisons must be licensed in their practicing state as an LPN with a minimum of one year experience in resident/patient assessment. Nurse Liaisons will maintain an appropriate level of knowledge about sources of payment and must understand the admission process from acute care to long-term-care. They will be able to travel 75% of the time on day trips to hospitals, etc. Nurse Liaisons have the ability to read and write well enough to follow written directions, do charting and resident assessments. Verbal communication and presentation skills are important to carry on conversations with hospital patients, residents, co-workers, and family members in an understandable manner. Nurse Liaisons are continually learning and growing by participating in educational sessions, in-service training and demonstrations. Current CPR certification. Physical requirements.

Summary of Position:

The Nurse Liaison works closely with the VHS facility and represents the facility to the medical/hospital community. The Nurse Liaison establishes and maintains positive relationships with primarily medical referral sources. The Liaison conducts physical assessments and/or screening of potential residents. This individual possesses an outgoing personality and demonstrates the ability to use discretion in exercising independent judgment.

  • Provide quality nursing services to meet the physical, psychological, spiritual, and restorative care needs of each resident, based upon your licensure and certifications.
  • Create and maintain an environment which allows each resident to adjust to their limitations and recognize the dignity and worth of each individual.
  • Participate in in-service educational classes and meetings directed at improving your ability to meet the needs of the residents.
  • Maintain and follow established safety precautions and infection control procedures when performing day-to-day responsibilities.
  • Know, understand, and perform responsibilities that meet regulatory and company standards.
  • Serve as an effective teacher, model, and consultant for other staff.
  • Be a reliable and positive member of the facility’s team.
  • Create a cooperative working relationship with other departments.
  • Represents the company with professionalism to the medical/hospital community referral sources (ie. care coordinators, discharge planners, social workers & physicians). Develops and maintains positive relationships to gain referrals.
  • Assesses potential residents/patients to ensure facility is able to provide appropriate level of clinical care
  • Communicates needs of the potential admission to the facility Admissions team based on admission screen & assessment.
  • Travels to hospitals to assess acute care residents, may need to do home/site visits
  • Meets with potential residents/patient and families to educate them of the facility services offered
  • Maintains contact and coordinates care with residents who return to the hospital from the facility to ensure facility care was satisfactory
  • Coordinates pre-authorization for managed care admissions
  • Serves as an active member of the facility census building team
  • Builds and maintains a strong working relationship with attending physicians in the area hospitals to promote skilled services, assist with knowledge of geriatric medications and assists with drug interchanges as needed
  • Coordinates and communicates resident/patient referral and approval process information
  • Ensures level of care approval obtained as indicated and OBRA screen (PASSAR) is completed if needed
  • Current understanding of all Medicare Regulations/Requirements
  • Tracks pre-admission referral activity daily/weekly
  • Reviews insurance contract content for need to obtain prior approval from authorized payer
  • Completes documentation in accordance with company policy and state &/or federal guidelines

Full time position. The hours will be 8:30a to 5:00p Monday through Friday.

Position pay depends on experience. Social Service background preferred, and insurance authorization experience is required.
