About the Role:
We are seeking a Hospice Chaplain to join our team in providing spiritual and emotional support to patients and their families during end-of-life care. As a Hospice Chaplain, you will be responsible for conducting spiritual assessments, providing counseling and support, and facilitating communication between patients, families, and healthcare providers. You will work closely with interdisciplinary teams to ensure that patients receive holistic care that addresses their physical, emotional, and spiritual needs. Your role will be critical in helping patients and families navigate the end-of-life journey with dignity and compassion.
Minimum Qualifications:
- Master's degree in Divinity, Theology, or related field
- Ordination or endorsement by a recognized faith group
- Clinical Pastoral Education (CPE) or equivalent training
- Experience providing spiritual care in a healthcare or hospice setting
- Excellent communication and interpersonal skills
Preferred Qualifications:
- Board Certification by a recognized chaplaincy certifying body
- Experience working with diverse populations and faith traditions
- Bilingual proficiency in English and Spanish
Responsibilities:
- Conduct spiritual assessments and provide counseling and support to patients and families
- Facilitate communication between patients, families, and healthcare providers
- Collaborate with interdisciplinary teams to develop and implement holistic care plans
- Provide education and resources to patients and families on end-of-life care and bereavement
- Maintain accurate and timely documentation of patient care
Skills:
As a Hospice Chaplain, you will use your strong communication and interpersonal skills to build relationships with patients and families, and to collaborate effectively with interdisciplinary teams. You will draw on your training in spiritual care and counseling to provide compassionate support to patients and families during a difficult time. Your ability to conduct spiritual assessments and provide appropriate interventions will be critical in ensuring that patients receive holistic care that addresses their physical, emotional, and spiritual needs. Your attention to detail and documentation skills will ensure that patient care is accurately and timely recorded.
