JOB SUMMARY:

This position is responsible for organizing and execution of program referral and intake list. The incumbent serves as the first point of contact for all referring agencies to the program. The incumbent is responsible for documenting all received referrals and reporting information to the program coordinator and team members during weekly staff meetings. The incumbent will be responsible for working with program staff and referral source to gather collateral information specific to referral case to determine eligibility for program enrollment. The incumbent will assign appropriate priority to eligible applicants and refer ineligible applicants to other community resources. The incumbent will coordinate appropriate referral or collaboration of transfer to other accountability court programs as needed. The incumbent will act as liaison between program and residential treatment programs to coordinate initial placement of participants and regular follow up for compliance. The incumbent will conduct Level of Service Inventory (LS-CMI) assessment to program participants at various intervals in program timeline to determine progress.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Organizes all program referrals and continuously documents information for weekly reports to program coordinator and team.

Works with referral sources and Clinical Evaluator to gather collateral information on all referred individuals to determine program suitability.

Assigns appropriate priority to eligible applicants and refer ineligible applicants to other community resources.

Attends weekly referral meetings and staff meetings as well as weekly court sessions as scheduled by the program.

Maintains accurate and comprehensive notations in case management system and makes available for review.

Creates initial file for all program referrals in electronic case management database and ensures all needed documentation is provided or attached.

Tracks all referrals in database and makes records available for review.

Assists in data collection as assigned.

Serves as liaison between residential treatment facilities and program to facilitate placement of participants and regular follow-up for compliance.

Coordinates with other accountability court programs to facilitate and receive transfers of program participants and/or referrals.

Conducts assessment (LS-CMI or other as assigned) at various intervals throughout program to determine participant progress and record findings in case management system.

Assists with drug screen collection as needed

Regular and predictable attendance is required

Performs other duties as required

MINIMUM QUALIFICATIONS REQUIRED:

Education and Experience:

Bachelor’s degree in psychology, counseling, sociology, social work, or related field. Familiarity with professional therapeutic or case management services to individuals with substance abuse and mental health issues is preferred. Experience working with the criminal justice population is preferred.

Incumbent must be able to work in a fast-paced environment of the courts under limited supervision. Must be familiar with HIPAA and Federal Confidentiality Rule 42CFR regulations.

Licenses and Certifications:

Must maintain all professionally related certifications.

Knowledge, Skills, and Abilities:

General knowledge of County organizational structure and complex Treatment Services operations;

Demonstrated proficiency in oral and written communications;

Extensive experience and skill in developing effective working relationships with other departments and the general public;

Extensive knowledge of mental health and substance use disorders, ASAM placement, and DSM-V;

Extensive knowledge of clinical assessment tools and diagnostic criteria based on DSM-V;

Demonstrated skill in developing and maintaining positive customer relations;

Extensive skill in operating a personal computer, various software applications, Internet, multimedia programs, and calculator.

ADA Minimum Qualifications:

Physical Ability: Essential functions of this position continuously require the ability to remain in a stationary position (sitting or standing); operate a computer and other office machinery, such as a calculator, copier, and printer. Incumbents must continuously be able to think analytically; handle stress and emotion, concentrate on tasks, remember names and other details; examine and observe details; make decisions, and adjust to change. Incumbents must also continuously direct others; meet deadlines; stay organized; use math/calculations and use a keyboard/type.

Essential Functions frequently require the ability to move about inside the office; and move/transport items up to 10lbs.

Essential functions occasionally require the ability to bend body downward and forward by bending spine and legs, and twisting at the waist, hips or knees; reach overhead; push or pull; and repetitively use hands/arms/legs. Incumbents in this position are occasionally required to be on-call and work irregular hours.

Sensory Requirements: Essential functions regularly require the ability to use effective verbal and written communication; and use of hand/eye coordination and fine manipulation to use equipment.

Environmental Factors: Essential functions of this position continuously require the ability to either work with others or work alone.
