Job DescriptionJob DescriptionMcGregor PACE is a Program of All-inclusive Care for the Elderly (PACE site), the alternative to nursing home placement providing services that address the medical, rehabilitative, social and personal care needs enabling seniors to remain safely at home, in the community.Home Health Aide / HHA, CNA or STNA Certification required; 1st and 2nd shift schedules at our East Cleveland, Warrensville Heights and Brooklyn locations$15.50/hourEssential Job Functions:1. Responsible for the day-to-day personal care and safety of the participant.2. Assists with activities of daily living (ADLs), including: personal care, toileting, bathing, ambulation, feeding, dressing and participating in recreational activities; encourages independence as appropriate.3. Responsible for light housekeeping, laundry, and assisting with meal preparation.4. Anticipates needs in order to carry through activities in organized, efficient manner; prepares all supplies and work area before beginning activities.5. May participate in interdisciplinary team meetings as requested; communicates relevant participant information to supervisor.6. Completes required documentation, including notes, etc., according to organizational guidelines and time frames, in a legible and complete manner.7. Reports participant status changes to supervisor and other staff as relevant.8. Validates competency in basic skills and personal care services, annually, and as otherwise required.9. Completes ongoing training program requirements, as per organizational policy, for the following essential elements: maintenance of a clean, safe and health environment; appropriate and safe recording temperature, pulse and respiration; and observation, reporting and documentation of patient status and the care or service furnished, as well as other elements consistent with assigned duties for specific participants, including dementia content.10. Follows organizational policy for call-off’s, completing time sheets.11. Assists with errands – grocery shopping or replenishing household goods.12. Understands and utilizes principles of infection control, including handwashing, bag procedures and care of the infectious patient.13. Innovative in providing service with available resources.Qualifications:
  • High school diploma or G.E.D. Requires Home Health Aide certification or CNA (certified nursing assistant) or STNA (state tested nursing assistant) education required. CPR certification preferred.
  • Prior experience working with frail, older adults, or persons with special needs, required; experience with dementia patients, desirable.
  • Patient, flexible in ability to communicate with all family members. Compassionate and understanding of the needs of frail older person.
  • Valid Ohio driver’s license or State of Ohio identification and reliable transportation
  • Ability to lift or move 50 – 75 lbs.

First shift; PRN weekends as needed
