Job Description:
Join our team as an Experienced Home Health Nurse. You'll need at least 1 year of home health nursing experience, proficiency in Axxess software, and outstanding charting skills. Your role is vital in ensuring top-notch care and precise documentation.

Key Responsibilities:

  • Conduct thorough health assessments at clients' homes.
  • Develop personalized care plans for optimal outcomes.
  • Administer medications and skilled nursing care.
  • Monitor patient progress and adjust care plans accordingly.
  • Educate patients and caregivers.
  • Maintain accurate electronic records using Axxess software.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Ensure compliance with home health regulations.

Qualifications:

  • Minimum 1 year of home health nursing experience.
  • Proficiency in Axxess software.
  • Strong charting and documentation skills.
  • Current nursing license in Illinois.
  • Excellent communication and interpersonal abilities.
  • Compassionate and dedicated to delivering high-quality care.
  • Ability to work independently and collaboratively.
  • Reliable transportation and valid driver's license.
