The Clinical Team Lead will report to the Senior Director of the Virtual Care Center and will work in partnership with the operations, clinical, and implementation teams. This individual will lead and optimize at-scale patient enrollment, patient, provider, and care team education, and orientation to Reimagine Care services at the client’s designated sites. This individual must strongly align with Reimagine Care’s core values, which focus on prioritizing patient and colleague respect and care, working collaboratively while demonstrating initiative and resourcefulness, and working with a sense of urgency to advance our innovation and impact on internal and external stakeholders.

Core Responsibilities:

  • Collaborate with multidisciplinary teams to deliver patient, provider, and care team education, optimizing patient enrollment, engagement, and satisfaction.
  • Take ownership by managing the end-to-end patient, provider, and care team engagement and satisfaction by resolving issues to standardize, refine, and improve processes.
  • Build and maintain positive working relationships and serve as an essential point of contact, clinical expert, and resource for patients, providers, and care teams.
  • Collaborate with internal and external stakeholders to create, review, and analyze patient enrollment, patient experience, and provider experience to identify trends, gaps, and areas for improvement.
  • Initiate, engage, and work collaboratively with the client’s operational teams, providers, and clinical teams to understand/evaluate business and clinical needs aligned to established goals and metrics.
  • Work collaboratively with members of the Virtual Care Center to follow all Reimagine Care policies, procedures, pathways, and communication and documentation best practices.
  • Understand and adhere to approved symptom management pathways, protocols, patients’ severity level, care pathway, and evaluation timeframes.

Education, Experience, and Skills

  • Education, Licensure and Experience:
    • Bachelor of Science in Nursing (BSN)
    • Current and valid Registered Nurse (RN) license in the state of practice
    • Minimum of 5 years of clinical experience in oncology
  • Skills:
