About the Opportunity

We are seeking per diem, part time and full time RN Case Manager candidates who are detail oriented and communicate effectively to aid in the care of our hospice patients. Tasks would include, but not be limited to, regular patient visits, care plan updates and revisions, medication administration and reconciliation, communication with interdisciplinary team members, and availability in emergency situations. Candidate must possess a current RN license in the state of Oregon. Previous experience in hospice nursing is a plus, but is not required. Job Summary

Provides direct palliative care to terminally ill patient and their families while functioning within a medically approved plan of care and demonstrates proficiency in triaging patient emergencies both in person and on the phone while effectively communicating patient needs and concerns to the correct member of the hospice team.

This position reports directly to the Director of Nursing

Job Duties and Responsibilities

Specific to Position:

  1. Follows an individualized Plan of Care established by the interdisciplinary team for each patient.
  2. Manages emergency care to patients encompassing coordination of all hospice disciplines during non-business hours and reports issues and significant changes to the RN Case Manager.
  3. Immediately reports any changes to the Physician, RN Case Manager or appropriate hospice team member for the patient.
  4. Reports unusual conditions, observations and pertinent information to the nurse.
  5. Provides intervention and support as needed related to terminal illness and loss within Scope of Practice.

  7. Assesses and address depression and fear associates with terminal illness
  8. Facilitates communication among patient, family and hospice team
  9. Capable of giving adequate report on patient’s condition to the Hospice Medical Director, members of the interdisciplinary team, other nurses and supervisors, as needed.
  10. Recognizes signs and symptoms of end-stage medical conditions/illnesses and implements planned nursing and medical interventions as delegated by the RN Case Manager or Hospice Physician. Coordinates medications, DME, and supplies necessary to complete the after-hours admission and keep the patient comfortable until business hours.
  11. Provides personal care as assigned by the Interdisciplinary Team, Chief Medical Officer, Hospice Physician, Director of Nursing, which may consist of but not limited to:
    1. Focused/limited nursing visits
    2. Telephonic triage
    3. Provide comfort measures
    4. Insertion/maintenance/removal of foley catheters
    5. Insertion/maintenance/removal of various intravenous and subcutaneous catheters for pain control/comfort per scope of practice and within individual’s competency set
    6. Evaluating IV site and check IV function
    7. Medication administration (via oral, transdermal, rectal, enteral, IV, SQ route) per scope of practice and within individual’s competency set
    8. Obtains accurate vital signs
    9. Performs standard diagnostic procedures including phlebotomy, venepuncture, and doppler.
    10. Assist with ambulation/transfers
    11. Dressing changes (including, but not limited to central line, IV, or SQ dressings, percutaneous dressings, ostomy dressings, tracheostomy dressings)
    12. Wound care
    13. Other duties as assigned.

Physical/Safety Requirements:

  1. Participates in emergency procedures, including disaster drills.
  2. Operation of equipment in a safe manner and shares responsibility for communicating safety violations or unsafe practices to management.
  3. Equipment Used: General office equipment (telephone, copy machine, computer, fax machine) and motor vehicles.
  4. Category 1 for potential exposure to blood/bodily fluids.

  6. Utilizes appropriate PPE, as needed.

  10. Must possess sight/hearing/speech senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position are fully met.
  11. Use appropriate body mechanics as needed for personal use.
  12. Utilize appropriate basic office equipment.

Requirements

Qualifications and Experience

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required to perform this job.

  1. Must be currently licensed as a Licensed Practical Nurse by the Oregon Board of Nursing.
  2. Prefer One-year experience as a Licensed Practical Nurse in hospice, home health or geriatric care
  3. Must be able to demonstrate basic LPN qualifications and successfully pass initial direct and indirect supervision competency items prior to direct care with a patient.
  4. Must maintain current CPR and First Aid Certification
  5. Must be able to speak, read and write the English language.
  6. Must have reliable transportation and maintain auto insurance.
  7. Successful completion of background check pursuant to ORS 181A.195.
  8. Read, write and understand the English language.
  9. Must be able to handle frequent interruptions, problem solve, and have exceptional follow through skills.
  10. Must be able to cope with emotionally charged situations encompassing patients, families, caregivers, and team members.
