All Care Homecare is a participant in the Group Adult Foster Care Program (GAFC). Responsibilities:

  • deliver professional nursing services in member's home setting and provide case management for primary patients
  • complete initial assessment and plan of care for home care members
  • prepare clinical and progress notes, submitting all required paperwork daily
  • coordinate services, informing the physician, management, and other personnel of changes in the member's condition and needs
  • ensure quality and accuracy of patient records and charts in compliance with Medicare and Medicaid standards
  • demonstrate compassion while working with ill, disabled, elderly and/or emotionally disturbed patients
  • respond promptly to telephone calls and emails
  • participate in continuing education programs to stay current on home health practices, laws and regulations
  • post hospital visits
  • monthly member visits
  • help to maintain confidentiality files
  • medication management services
  • perform other duties as assigned

Education and Experience Requirements:

  • RN with current MA license, BSN preferred
  • one or more years of current nursing experience in certified home care is preferred, but not required
  • ability to sit, stand, bend, lift, kneel, climb stairs and move patients safely as required
  • ability to work in the patient's home environment as well as the office
  • demonstrate strong verbal, written and interpersonal skills to communicate effectively
  • must have reliable automobile, valid driver's license, and current auto insurance
  • fluent in English with skills in a second language a plus
  • computer skills sufficient to capture notes and required documentation
