A DAY IN THE LIFE OF A PATH HOMELESS OUTREACH CASE MANAGER

In this part-time role, your day begins with a sense of purpose as you set out to connect with individuals facing homelessness and mental health challenges. As a PATH Homeless Outreach Case Manager, you are their guide, their advocate, and their source of support. From conducting assessments to crafting personalized care plans, every moment is dedicated to empowering those in need and helping them find their path to stability and hope.


QUALIFICATIONS

  • Ability to work with individuals with severe and persistent mental disabilities
  • Understanding of psychotropic medication effects
  • A passion for making a difference in the community

Preferred Qualifications:

  • Bachelor's degree in psychology or social work
  • Experience working with homeless populations

WORK SCHEDULE

This is a part-time opportunity, working approximately 20 hours per week.
