We are searching for candidates in the Polk County area to service our Polk City clients. Provide excellent care to our existing client ensuring their needs are met with professionalism and compassion. Need to Support 12 hours per week, Mon, Wed and Fri from 1-5pm and possibly another day (TBD)

- Assist with Activities of Daily Living (ADLs):
- Provide support with bathing, grooming, dressing, and personal hygiene.
- Assist with mobility, transfers, and ambulation.
- Medication Reminders:
- Provide reminders for medication according to prescribed schedules.
- Support clients in managing their medication routines.
- Meal Preparation and Nutrition:
- Assist with meal planning and preparation, adhering to dietary requirements.
- Monitor and encourage proper nutrition and hydration.
- Companionship and Social Interaction:
- Engage clients in meaningful conversations and companionship.
- Accompany clients to appointments and social activities.
- Health Monitoring:
- Observe and report changes in clients' health status to the care team.
- Document care provided and any notable observations.
- High school diploma or equivalent preferred.
- Previous experience as a caregiver or similar role preferred
- Certification in HHA / CNA & in CPR and First Aid preferred.
- Compassionate and empathetic attitude.
- Strong communication and interpersonal skills.
- Ability to follow care plans and work collaboratively within a team.
- Ability to follow the WellSky App, training and guidelines available.
