POSITION SUMMARY

This position provides personal care and/or home management tasks to clients requesting and eligible for Caregiver services, under the direction of the RN.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Practice within the scope of training. license, certification, or registration, where applicable, and as required by law and regulation.
  • Performs tasks as delegated and directed by the registered nurse.
  • Assist clients with Activities of Daily Living and/or Instrumental Activities of Daily Living, including but not limited to:
    • Assisting with ambulation, transfers, or exercises
    • Assisting with bathing, dressing, and grooming
    • Assisting with feeding,
    • Assisting with toileting, and incontinence care,
    • Housekeeping,
    • Meal preparation,
    • Safe transportation,
    • Medication reminders,
    • Pet care,
  • Maintain general order, safety, and cleanliness of Client area.
  • Communicate any changes in Client condition or occurrences to registered nurse.
  • Maintain chart documentation of care provided in accordance with the written plan of care.
  • Practice safety and infection control methods.
  • Demonstrate proper transfer and lift techniques using good body mechanic techniques.
  • Maintain confidentiality & privacy of any PHI per HIPAA requirements.
  • Exhibit competency in the observance of Universal Precautions by:
    • Knowing standard precautions
    • Washing hands before and after patient care,
    • Wearing gloves and other PPE as required.

MINIMUM QUALIFICATIONS

Experience:

  • 1 year caregiving experience, preferred

Licensures & Certifications: (if applicable)

  • Valid NC CNA certificate
  • Valid driver’s license and proof of automobile insurance

SUMMARY OF PHYSICAL REQUIREMENTS

  • Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
  • Flexion/Extension of arms/wrist
  • Hand dexterity to include gripping, pushing/pulling and writing
  • If assigned to hands-on client, ability to assist with transfers/repositioning,
  • If assigned to hands-on client, lifting/carrying of 50 lbs

OTHER DUTIES

  • Report to work on time, ready for shift, and appropriately dressed for role.
  • Consistently demonstrate Four Seasons values and competencies
  • Complete required orientation and on-going training
  • Participate in ongoing education and training offered by the agency and/or outside resources as required for role.
  • Practice professional boundaries and behaviors and communication.
  • Practice personal and environmental safety.
  • Perform other duties as assigned by register nurse, Office Manager, or Director.
