Registered Nurse Case Manger - Hospice Homecare 4- 10hr shifts

Your Role:

  • Compassionate Care: Provide expert nursing care and emotional support to patients and their families.
  • Holistic Assessments: Conduct comprehensive assessments to understand and address patients' physical, emotional, and spiritual needs.
  • Personalized Planning: Develop tailored care plans in collaboration with patients and their loved ones, ensuring comfort and dignity throughout their hospice journey.
  • Coordination Services: Coordinate services and resources to optimize patient care and support.
  • Empowerment Education: Educate patients and families about hospice services, disease progression, and end-of-life care options.
  • Clinical Excellence: Administer medications, manage symptoms, and adhere to clinical protocols with precision and empathy.
  • Team Collaboration: Work closely with physicians, nurse practitioners, social workers, and other healthcare professionals to enhance patient outcomes and quality of life.

Qualifications:

  • Registered Nurse (RN) license in good standing.
  • Excellent communication and problem-solving skills.
  • Ability to work independently and manage time effectively.
  • A compassionate and patient-focused approach to care.
