POSITION DESCRIPTION:

The Community Health Worker position is responsible for establishing connections between community residents, health care system, and social care resources to improve life circumstances and health outcomes. This position will help individuals navigate physical health, behavioral health, and social services systems by addressing social determinants of health and serving as a bridge between individuals, healthcare providers, and social service agencies. The Community Health Worker works in both clinical and community-based settings, including the clients’ homes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Complete comprehensive assessment for assigned clients, provide referrals as appropriate, and follow-up on outcomes and next steps.
  2. Contact clients (phone, text, email, home visit, etc.) regarding need for health services and schedule appointments for health screening, well-visits, and follow-up appointments.
  3. Assist clients in accessing and understanding health information provided by their medical and social service providers.
  4. Assist clients with completing paperwork related to health and social determinants of health needs such as applications and registration forms.
  5. Conduct home visits as needed to provide education, follow-up, support, screening, etc.
  6. Collaborate with internal and external partners through an integrated approach to patient care, care coordination, and patient engagement.
  7. Participate in community events to promote services and healthy behaviors.
  8. Maintain accurate and up-to-date client records by documenting services and client interactions in the CHW database system.
  9. Complete community health worker training and subsequent assigned trainings.
  10. Perform other duties as assigned.

  7. Moderate comfort level using computer and technology.

EDUCATION AND EXPERIENCE:

  1. High school diploma or GED, valid driver's license, insured and reliable transportation; and lived experience or extensive relationship with the community is required.
  2. Experience in community outreach, peer counseling, health promotion, or similar role, customer service experience, healthcare/medical office experience, and/or data entry experience is desired.

MEDICAL HOME STATEMENT:

Be aware and understand expectations and responsibilities of the Patient Centered Medical Home. Understand role, complete relevant duties, and the requirement to maintain highest level of proficiency within scope, as part of this team-based approach in the patient care continuum.

PHYSICAL DEMANDS:

Light physical activity performing non-strenuous daily activities of an administrative nature. Must be able to lift up to thirty (30) pounds.

WORK SCHEDULE:

Normal work hours for this position are Monday through Friday dayshift. Attendance at off site meetings will be required with no overnight stay. Local travel required. Valid driver’s license and insured automobile required. Occasional out-of-town travel may be required.
