Transition Coordinator

The Lutheran Home is looking for a Transition Coordinator to join our team. In this rewarding career, you will be a front line advocate for our organization promoting our community and services as you facilitate the successful transition of residents to our health center from both community and hospitals.
Responsibilities may include but are not limited to:
- Review referrals and ensure that the facility can meet the needs of the potential patient.
- Meet new admissions/families at the hospital, & inquire about any special needs or helpful information that would be beneficial to the clinical team.
- Conduct tours of the facility with potential residents/families.
- Ensure that all needed equipment is ordered and/or available prior to patient’s arrival.
- Upload referral information into EHR and communicate with the business office to verify insurance information and initiate insurance pre-authorizations as needed.
- Market our facility and services to social workers, case managers, physicians, nursing staff, potential customers, etc.
In this salaried position you will enjoy a typically Monday thru Friday schedule working 8:00am – 4:30pm and take part in our many benefits including:

Education, Experience:
