The Hospice at HCS RN provides attentive, compassionate, and comprehensive nursing services to patients in a private home or residential facility setting. This is a full-time position with benefits. **This position offers a $5,000 Retention Bonus**


As part of the hospice interdisciplinary team, this position also:

  • Collects data and conducts focused nursing assessments of the health status of clients;
  • Participates in the development and modification of the comprehensive plan of care for all types of clients;
  • Maintains safe and effective nursing care rendered directly or indirectly;
  • Participates in health teaching and counseling to promote, attain, and maintain the optimum health level of clients;
  • Serves as an advocate for the clients by communicating and collaborating with other health service personnel;
  • Participates in the evaluation of client responses to intervention;
  • Provides input into the development of policies and procedures;
  • Communicates effectively with supervisor, staff, patients and families;
  • Plans workload according to client needs and location, completing four to five patient visits per day;
  • Exhibits full compliance with the agency policies concerning infection control, management of hazardous waste and client education/reinforcement; and
  • Conveys a positive image of the agency through personal appearance, attitudes and behaviors.


Requirements:

  • Graduate of a state-licensed basic nursing program;
  • Two or more years of nursing experience, preferably in a home care setting;
  • Possession of a current State of New Hampshire RN license;
  • Current cardiopulmonary resuscitation (CPR) certification;
  • Valid driver’s license and access to an automobile during working hours that meets the State of New Hampshire registration regulations and auto liability insurance;
  • Able and willing to learn to utilize a portable electronic field device to manage electronic patient health information (ePHI) remote access in accordance with HCS policies & procedures;
  • Sensitivity in human relationships. Able to cope with family emotional stress and tolerant of individual lifestyles. Skill in using psychological principles in both one on one and group situations. Knowledge of measures to comfort the dying. Emotional stability, flexibility, and respect for various religious and spiritual beliefs; and
  • Must be willing to become Hospice and Palliative Care certified.
