Successfully promote and support the mission of Catholic Community Health. The LPN is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. EEO


ESSENTIAL FUNCTIONS:


  1. LPN provides skilled hospice nursing care to patients as directed by the Registered Nurse and IDT plan of care, in accordance with agency policies.

2. Implements current nursing practice following a comprehensive assessment and the plan of care.


3. Provide accurate and timely documentation of patient services to reflect the plan of care.


4. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.


5. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.


6. Uses and prepares equipment and supplies effectively, adhering to aseptic technique.


7. Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.


8. Participates in personal, professional growth and development. Also participates in organization's quality assessment

performance improvement teams and activities as requested by direct supervisor.

Requirements:

MINIMUM POSITION REQUIREMENTS:


1. Registered in the State with current licensure to practice professional nursing in the State.

2. Experience in a hospice or related health care organization.

3. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.

4. Has excellent observation, verbal and written communication skills.

5. Demonstrate a passion for the mission of Catholic Community Hospice

6. Must be willing to perform by the agency’s values


PHYSICAL REQUIREMENTS:

  1. Prolonged periods of sitting while driving, at a desk working on a computer, or standing, bending and moving.
  2. Must be able to lift up to 10 pounds at times.
  3. Must have the physical ability to complete assignments in a variety of environments: homes of those we serve, extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
  4. May be exposed to infectious diseases
  5. 5. Must have physical ability to make visits to a variety of environments that may include walking on rough terrain and climbing steep stairs
  6. 6. Must have physical ability to effectively communicate with others
  7. 7. Must have the physical ability and appropriate licensure for driving
