Requirements:

  • 3 years of Clinical Management in a Home Health setting or similar environment
  • AZ Fingerprint Clearance Card
  • Current licensure in the State of Arizona or Multistate license
  • Up-to-date Negative TB Test Result
  • BLS CPR Certification
  • Driver's License and Auto-Insurance
  • Immunization/Vaccination Records (Covid19, Flu Shot, Hep B, MMR, TDaP, Varicella)


Others:

  • Prior supervisory experience (clinical oversight, recruitment, competency evaluations, etc.)
  • Knowledge of Electronic Medical Record systems
  • Possess and maintain a current CPR certification
  • Home care and trach/vent/tube experience is a plus
  • Strong assessment and organizational skills
  • Demonstrated success of working independently while taking on increased responsibilities
  • Ambition to grow and advance beyond current position
  • Solid communication and PC skills.
  • Must be able to do field work.


ABOUT THE POSITION

  • Ensure adherence to policies and procedures
  • Revision of policies and procedures as needed for adherence to updated regulatory standards
  • Assigns and directs staff with appropriate skills to perform duties. Coaches, directs and encourages staff through use of formal and informal means. Provides leadership planning for staff education programs, administrative/staff meetings, in-services, and encourages continuous education and staff development for all team members.
  • Create a warm, organized and welcoming office grounded in customer service
  • Sets positive, attainable expectations, objectives and goals for the agency.
  • Accountable for internal case management
  • Provide clinical insight and focus on clinical competency of the staff
  • Assuring current and accurate clinical records in compliance with documentation requirements and program specific rules for coverage and qualifying criteria. Assures clinical records are maintained in their original state and not improperly altered or fabricated.
  • Ensuring agency and clinicians are compliant with all Federal, and State regulations as well as standards of practice for the nursing and therapy teams.
  • Responsible for reporting goals and key performance indicators to Administrator and governing body on an ongoing basis.
  • Maintains ongoing liaison between the Administrator and Agency Staff.
  • Ensures adequate staff development, performance, evaluations, and education through in-services and on the job training.
  • Participates in the selection of orientation of adequate, competent staff.
  • Assures appropriate staff supervision during all operating hours.
  • Supervises and evaluates client satisfaction survey reports.
  • Recruits employees and retains qualified personnel to maintain appropriate staffing levels.
  • Handles necessary employee corrective action and discipline issues objectively, in consultation with HR.
  • Ensures implementation of a performance improvement program to drive quality care, positive patient outcomes, and address negative trends. Ensures issues and deficiencies identified during internal or external surveys or audits are addressed and corrected.
  • Participates in on call process, including admin on call, preparing on call schedule, supports the on-call process.
  • Monitors and evaluates employee performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed. Develops employees through coaching, mentoring, and formal/on the job training and development opportunities.
  • Sets a field schedule for supervisory purposes
  • May have to fill in for clinical visits
  • May be appointed to assume the role of the Administrator during a temporary absence and/or vacancy.
  • Performs other related duties as assigned.
