Housing & Outreach Advocate - Harbor House

Schedule: Monday - Friday, 8 am - 5 pm
Under general direction from the client services supervisor, the housing and outreach navigator provides advocacy and case management towards independent living to clients who reside in shelter home. Receives prospective clients and determines safety, eligibility for service.
Accountabilities
- Receives prospective clients and determines eligibility for services.
- Admits and exits clients through established procedures.
- Interviews clients to assess independent living needs and advocacy needs. Develops plan to assist shelter client’s with obtaining housing. Advises client in areas such as budgeting, life skills, housing, employment or other source of income, and goal setting to eliminate barriers to obtaining safe and stable housing. Provides safety planning related to housing throughout their stay.
- Provides advocacy and supportive services to families as they transition out of shelter & into their new home.
- Provides additional services to families on an outreach basis, as needed or at the direction of their supervisor.
- Coordinates a spectrum of community resources and refers clients to subsequent services. Maintains professional contact with community resources and advocates client needs.
- Records assessments, case management notes, service plans, vouchers, CQI, medications, release of information, exit summaries and referral services. Follows up with clients to assess progress and provide continuing service as appropriate. Closes client files after client’s exit.
- Enters program demographic and service data in the Catholic Charities computer system.
- Manages daily requirements of shelter.
- Maintains shelter safety and responds to issues appropriately as they arise.
- Provide all 13 Core Domestic Violence Services in accordance with our accreditation and agency policies.
- Receives and acknowledges donations to the shelter, if necessary.
- Records Incident Reports and Work Orders in Catholic Charities computer system, as required.
- Shares in on-call responsibilities
- Attends domestic violence trainings as required by KCSDV and Catholic Charities Wichita.
- Demonstrates an understanding of, and support for, the mission of Catholic Charities and acts as a representative of Catholic Charities in this role.
- Performs other related assigned duties.
Requirements
- General
- Must demonstrate an acceptance and respect for cultural diversity, sexual orientation, and religious backgrounds.
- Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with a diverse clientele, staff, community partners, and allied professionals.
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must have valid Kansas Driver’s License with clean driving record, reliable transportation w/ valid vehicle insurance and ability to transport victims in personal vehicle, as needed.

- Education
- Undergraduate degree in social work or social services or other related field.
- Certificate, Registration, Licensure
- Class C driver's license, motor vehicle, and insurance required.
- Experience
- Professional experience or training in counseling, domestic violence, economically disadvantaged, or other social issues.
- Skills and Abilities
- Able to use a personal computer via mouse and keyboard.
- Familiarity with Windows and Microsoft products.
- Exposures
- Controlled inside work environment with noise/vibrations from general office equipment. Frequent exposure to other environments such as private homes, doctor offices, etc.
- Minimal exposure to fumes/dust/odors.
- Work Type: Light, exerting up to 30 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
