The Supervisor is responsible for overseeing the day-to-day staff activities. Will interact, coordinate, and facilitate activities with all disciplines, departments, and appropriate regulatory agencies.Responsibilities
  1. Responsible for ensuring the timely and comprehensive completion of initial and re-assessments with particular attention to the nursing component, including specific short- term nursing goals in each individualized plan of care.
  2. Maintains acceptable standards of nursing care, based on program specific regulatory requirements as well as Joint Commission and the organization’s standards.
  3. Assist with interviewing and hiring of clinical support and nursing staff.
  4. Responsible for competency training, in-services, and assessments including but not limited to venipuncture, Infection control, Injections, immunizations, protocols, and nursing documentation as appropriate.
  5. Assists with preparing for various private, county, and state audits.
  6. Responsible for the implementation, monitoring, and maintenance of all logs, including monitoring, administering, and recording ordered medications.
  7. Responsible for ensuring the ordering and maintaining of adequate supplies and equipment. May delegate to staff members as needed.
  8. Responds to medical emergencies and will take the lead in the management of the situation as indicated.
  9. Screen and triage “walk-in” and “telephonic patient contacts” as needed.
  10. Provides health education and counseling to the participant and family when indicated by the plan of care.
  11. Acts as liaison between departments, in addition to community providers, specialists, institutions, home health agencies, DME vendors, and appropriate regulatory agencies.
  12. Supports and enforces infection-control policies and procedures and supports the Quality Improvement Programs, and Utilization Management (PACE) programs/strategies.
  13. Participates as an active member of the inter or multi-disciplinary team to assess the nursing need of the patient.
  14. Accurately assesses and documents patient status to support the regulatory review and approvals of initial enrollment (LOC for PACE).
  15. Ensures annual review of program policies and procedures, modifying as appropriate and consistent with appropriate standards.
  16. Supervises, coordinates, and ensures efficient operations in the clinical setting as appropriate.
  17. Completes subordinates’ performance evaluations in a timely manner with coaching and appropriate ongoing communications with staff.
  18. Performs other related duties as assigned.

  3. Strong team-building skills, and excellent customer service.
  4. Knowledge of program regulations and preventative measures for “at risk” participants.
  5. Excellent health assessment skills relating to geriatrics.
  6. Detail oriented and thorough.
  7. Excellent oral and written communication and presentation skills.
  8. Ability to multi-task with excellent prioritizing skills.
  9. Ability to manage crisis situations and maintain structure and organization.
  10. Computer literate in Microsoft Word/Excel/PowerPoint.
  11. Knowledge of clinical care practices, operations, and local, state and federal regulatory standards.
Qualifications
  1. Graduation from an accredited school of nursing and current valid license by the State Board of Nursing.
  2. A minimum of two years’ experience in geriatric nursing preferred and minimum one year experience in the supervision and management of staff in a healthcare environment.
  3. Bilingual English/Spanish, Tagalog, or Mandarin preferred.
