Job Overview:


Wasatch Crest Treatment Services is actively seeking a dedicated Case Management Coordinator to join our team. The Case Management Coordinator will play a crucial role in maintaining strong connections to ensure that each client receives the support and information necessary to build a life in long-term recovery. The Case Manager is responsible for creating and developing discharge and aftercare plans, referent communication, family communication, and transitions through a full continuum of care. This role supports clients in planning for and addressing all non-clinical areas of their life. It is their responsibility to ensure that clients, referents, and families receive world class hospitality and care.

 

Duties:

  • Monitor Clients physical and emotional well-being and report unusual behavior or physical ailments to clinical staff.
  • Develop or teach strategies to promote client wellness and independence.
  • Take and record measures of Clients physical condition, using devices such as thermometers or blood pressure gauges.
  • Encourage clients to develop work skills and to participate in social, recreational, or other therapeutic activities that enhance interpersonal skills or develop social relationships.
  • Assists with new client admissions and discharges including property searches, documentation, and client orientation
  • Maintains awareness of client safety at all times
  • Facilitates daily chores with clients
  • Assists with urine analysis drug screening when directed
  • Provides reliable and safe transportation to appointments and activities
  • Contact Clients’ relatives to arrange family conferences.
  • Accurately charts observations on client at end of shift using correct charting format
  • Documents unusual or high-risk behavior and reports concerns to supervisor
  • Participate in routinely scheduled Case Manager trainings
  • Willingness to cover shifts due to illness, vacations, holidays, etc. (if needed)
  • Completes other duties as assigned by supervisor


Qualifications:

  • Bachelor’s degree or higher preferred.
  • Previous experience serving individuals with mental health diagnoses, or substance use is preferred
  • Must have a valid driver’s license in Utah
  • Current Basic Life Support and First Aid certification required, within 90 days of hire certification must be completed.
  • Must be physically and mentally able to assist in an emergency situation
  • Must have a TB screen within the last year or able to get one within 14 days of hire
  • Ability to interact in a clinical environment while maintaining appropriate boundaries
  • Effectively communicate with professionals in the legal system.

  • Must be able to establish strong rapport with clients, referents, and families.
  • Ability to comfortably have financial discussions with clients and families.
  • Must have excellent computer and technology skills.
