ABOUT THE ROLE:

We are seeking a dedicated and compassionate Hospice Licensed Practical Nurse (LPN) to join our team. This full-time position involves working closely with an RN case manager to provide exceptional care to patients in both facility and home settings.

- Administer medications and treatments as prescribed by the physician

- Monitor patients' vital signs and report any changes to the clinical team

- Maintain accurate patient records and documentation.

- Educate patients and their families about care plans and medications.

- Ensure compliance with all healthcare regulations and protocols

MINIMUM POSITION REQUIREMENTS:

1. Registered in the State with current licensure to practice professional nursing in the State.

2. Experience in a hospice or related health care organization is preferred but not required.

3. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.

4. Has excellent observation, verbal and written communication skills.
