St. Joseph's Addiction Treatment and Recovery Centers

 

Location: Inpatient Facility – Saranac Lake, NY

 

Position: Full-time Aftercare Counselor

 

Schedule: Sunday through Thursday, 8:00am-4:30pm

 

Pay Range: $45,000/year to $100,000/year

 

Experience and Work Knowledge: Telephone skills, typing skills, standard business practices, able to work independently, computer skills and knowledge of chemical dependency helpful.  CASAC preferred.

 

Physical Demands: No extraordinary physical demands required.

 

Duties and Responsibilities:


  1. Develop and maintain therapeutic relationship with clients.
  2. Actively participate in clinical supervision.
  3. Function as a supportive multi-disciplinary team member
  4. Complete thorough admission procedure for clients.
  5. Assists with program service delivery in absence of Primary Counselors to include admissions, assessments, treatment plan coordinator and group/individual coverage.
  6. Coordinating Resident transportation for discharge.
  7. Assist Residents in completing Aftercare referral applications.
  8. Conduct aftercare sessions.
  9. Perform discharge meetings.
  10. Compiling and maintaining of referral files.
  11. Collaborating with the Utilization Review Team for coordination of resident funding in preparation for discharge.
  12. Responsible for facilitation if didactic lectures as assigned.
  13. Facilitate phone screenings.
  14. Follow up with referrals to establish placements for aftercare.
  15. Responsible for additional duties as may be assigned by the Supervisor.
  16. Display professional image, appearance, and behavior reflective of SJRC Mission, policies and procedures.
  17. Provide weekend and holiday coverage.
