Your day as a PCHH RN Case Manager-SedaliaAs a new PCHH RN Case Manager at Katy Trail Community Health in Sedalia, you can expect to provide individualized patient care plans, coordinate services, and collaborate with a multidisciplinary team to ensure the best outcomes. Your days will be filled with patient assessments, care coordination, documentation, and follow-ups to guarantee continuity of care. With a Monday to Friday schedule from 8 am to 5 pm, you will have the stability and work-life balance to excel in your role. What you need to be successfulTo excel as a PCHH RN Case Manager at Katy Trail Community Health in Sedalia, you must maintain a valid RN licensure in Missouri, showcasing your commitment to professional standards. With at least three years of experience in a clinic setting, you will bring valuable expertise to the role. While experience in a community health center is preferred, your ability to adapt and learn quickly is essential. Additionally, possessing current CPR certification is crucial to ensure you can respond effectively in emergency situations. Your proficiency in utilizing various software and tools will enable you to streamline patient care and documentation processes, contributing to the efficiency and success of our healthcare services.
Knowledge and skills required for the position are:
- Maintains valid
- current RN licensure in the State of Missouri
- A minimum of three (3) years' experience in a clinic setting
- Experience in a community health center setting
