SUMMARY

The Outpatient Nutritionist performs a variety of specialized duties related to the nutrition counseling of patients/families for diet education. This may include but are not limited to diabetes education, weight management for pediatric and adults, cardiovascular disease, renal disease, malnutrition/weight gain, cancer and nutrition therapy, gastrointestinal disorders and nutrition therapy, sports nutrition or healthy diet.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Interviews patients to complete a dietary assessment which may include obtaining a diet history, patient’s weight, height (as needed), BP, heart rate, exercise habits, reviewing emotional/social issues as part of the diet counseling process and documents any pertinent information obtained in the Electronic Health Record.
  • Coordinates and implements an individualized nutrition plan of care, including involving the client/family in the development of the client’s goals and collaboration with other health care professionals as necessary and revising the nutrition plan of care as needed.
  • Makes appropriate referrals to other services within Aunt Martha’s, as indicated.
  • Counsels patients/families on medical nutrition/diet therapy principles and restrictions using evidence-based nutritional education materials
  • Demonstrates evidence of continuing educational development through participation in appropriate seminars, classes and/or in-services; as well as self-study materials.

POSITION REQUIREMENTS/EDUCATION:

Registered Nurse or Bachelor’s or Master’s Degree in Health Science with an emphasis in nutrition or closely related degree. It is required that the individual has experience or course work in working with those who have chronic conditions, maternal and child nutrition, lactation, nutrition assessment and nutrition education.

  • Strong attention to detail and accuracy.
  • Knowledgeable about community resources
  • Experience in working with Microsoft Office programs essential.
  • Must be able to read, write, and speak English.

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty, responsibilities, essential functions and key competencies satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Principles of life-cycle nutrition - Understands normal nutrition issues for pregnancy, lactation, the postpartum period, infancy, and early childhood.

Anthropometric technique - Understands the importance of using appropriate measurement techniques to collect anthropometric data.

Communication - Knows how to develop rapport and foster open communication with participants and caretakers.

Multicultural awareness - Understands how sociocultural issues (race, ethnicity, religion, group affiliation, socioeconomic status and world view) affect nutrition and health practices and nutrition-related health problems.

Critical thinking - Knows how to synthesize and analyze data to draw appropriate conclusions.

OTHER QUALIFICATIONS:

  • Current driver’s license with clear driving record
  • Current auto insurance
  • Ability to work evening hours and weekend hours based on program needs
  • Current Healthcare Provider Cardiopulmonary Resuscitation Certification (CPR/BLS).
