Key Responsibilities
- Patient Encounter Documentation: Accompany healthcare providers to document medical information in real-time during patient visits.
- EHR Management: Utilize EHR systems to input and manage patient data in compliance with confidentiality standards.
- Chart Review: Review and organize patient charts for completeness and accuracy.
- Effective Communication: Facilitate communication between healthcare providers and team members for timely information exchange
- Data Entry and Coding:Enter ICD-10 and CPT codes for diagnoses and procedures accurately.
- Documentation Queries Liaison: Resolve documentation-related queries between the coding team and providers.
- Morning Huddle Preparation: Research patient's historical charts to enhance morning huddle discussions.
- Quality Metrics Management: Measure, analyze, and report healthcare quality metrics to improve patient outcomes.
- Population Health Optimization: Analyze health and coding strategies to assist in the management of at-risk patients.
- Evidence-Based Research: Stay updated on medical protocols to support the care team.
- Maintain Confidentiality:Ensure compliance with patient privacy regulations (e.g., HIPAA).
What You'll Bring
Knowledge, Skills, and Abilities:
- Bachelor's degree in healthcare or related field preferred.
Education, Experience, Licensure, or Certification Requirements:
- Minimum of 2 years of clinical experience.
- Familiarity with medical terminology and prior scribing experience is preferred.
- Proficient in EHR systems, strong typing skills, and computer proficiency.
- Excellent communication skills and ability to thrive in a fast-paced environment.
- Strong organizational skills and problem-solving abilities.
- Willingness to work across various Neighborhood Centers.
- Proficiency in English and Spanish is essential.
