Job Summary:
As the Director of Case Management, you'll oversee department activities, ensure compliance, and coordinate patient care processes for optimal outcomes.
Responsibilities:
- 📊 Direct and evaluate departmental operations to achieve performance objectives.
- 🤝 Establish working relationships with key stakeholders for effective care coordination.
- 📋 Assess department performance and implement improvement activities.
- 🔍 Ensure compliance with policies, SOPs, and care coordination processes.
Qualifications:
- 🎓 Bachelor's degree required; Master's degree preferred.
- 💼 RN licensure or LCSW required.
- 🏥 5-7 years' experience in acute care Hospital Case Management.
- 👩️💼 2+ years' experience in case management leadership preferred.
