Responsibilities:

- Provide exceptional nursing care to patients in the comfort and familiarity of their own homes.

- Administer medications and treatments as prescribed by physicians, ensuring the highest standards of patient safety and well-being.

- Monitor patient vital signs, assess health status, and communicate any changes or concerns to the healthcare team.

- Collaborate with interdisciplinary team members to develop personalized care plans tailored to each patient's unique needs.

- Educate patients and their families on effective healthcare management and disease prevention strategies.


Requirements:

- Valid and active LPN license in Florida, demonstrating your commitment to excellence in nursing practice.

- Minimum of 1 year of experience in home health or a related field, showcasing your expertise in providing quality care in a home setting.

- Proficiency in nursing assessments and care planning, allowing you to deliver personalized and patient-centered care.

- Excellent communication and interpersonal skills, enabling you to establish rapport and build trusting relationships with patients and their families.

- Ability to work independently while also collaborating effectively within a multidisciplinary team.

- Valid driver's license and reliable transportation to travel to patients' homes.
