POSITION SUMMARY

The Pharmacist will be responsible for quality pharmacy services, collaborating with primary care providers and staff to

improve the health of FHCN patients. The Pharmacist assures compliance with all regulations of the SC State Board of

Pharmacy, and works to achieve the goals, objectives and budget established by FHCN’s Board of Directors and Senior

Leadership Team. Pharmacy services will include dispensing medications, assisting FHCN personnel and patients with

drug information and identification, developing and sustaining productive patient relationships, evaluating and resolving

potential drug therapy problems identified through any and all available sources, patient counseling, prescription

verification, adherence to state and federal pharmacy laws, precepting students and interns, supervision of pharmacy

technicians while on-duty, and participation in projects as assigned. FHCN manages patient care using a team-based

approach in our interactions with patients and working to achieve stated objectives and outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES (Included but not limited to the following);

  • Knowledge of Board of Pharmacy regulations
  • Knowledge of 340b program
  • Ability to work with primary care providers to maximize cost effective prescribing patterns, improve clinical processes, and achieve clinical standards across the population of patients served by FHCN.
  • Proficiency in computers, including ability to learn Electronic Medical Records and pharmacy software, also use phones, fax machines, printers, scanner, & copiers.
  • Ability to carry out supervisory responsibility in accordance with the organization’s policies and applicable laws.
  • Ability to work in a fast paced office environment with frequent interruptions.
  • Clinically competent.
  • Knowledge and experience of ordering, inventory and cost management activities.
  • Ability to effectively communicate, verbally & in writing, with all levels of staff and patients.
  • Ability to keep customer service and the mission of the organization in mind when interacting with clients, co-workers, and others, promoting a positive image for FHCN and fostering an ethical work environment.
  • Ability to maintain strict confidentiality with medical information and conform to HIPAA regulations.
  • Must possess strong problem-solving skills and effective time management skills.
  • Ability to understand and respond effectively and with sensitivity to special populations served by FHCN. Special populations include, but are not limited to, those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, disability status, migrant, homelessness, seasonal workers, and the uninsured.
  • Practice pharmacy in compliance with all federal, state, and local laws and regulations, as well as FHCN policies.
  • Assign, coordinates and oversees Pharmacy technician’s tasks
  • Ensure that all pharmacy technicians are working in compliance with all federal, state and local laws and regulations, as well as FHCN policies.
  • Review prescriptions for accuracy, appropriate dosage, directions, drug interactions and, and contact the provider ifquestions arise.
  • Counsel patients on drug usage, storage, side effects and drug interactions in accordance with federal and state pharmacy laws.
  • Assess allergy concerns and communicate to patients and prescribers
  • Process and dispense prescriptions in a timely and efficient manner
  • Bill Third Parties and handle rejections
  • Maintain accurate records of controlled substances
  • Remove outdated and damaged drugs from the pharmacy inventory
  • Process recalls according to policy and procedures.
  • Oversee daily ordering of medications.
  • Establish and maintain good working relationships with staff, physicians and other health care professionals
  • Keep pharmacy secured.
  • Answer and make patient and prescriber calls
  • Make sure Home/ Site Delivery patients receive medications in a timely manner.
  • Communicate with pharmacy director any issues within the pharmacy as well as deliveries to the sites.
  • Assist with training of pharmacy staff
  • Assist pharmacy director in other duties as assigned.

  • May need to assist with moving or lifting >30 pounds.
  • Requires visual acuity to perform tasks requiring close vision and ability to adjust focus.
  • Required to be able to travel to meetings outside the organization’s facility.
  • Required to be able to work evenings and weekends if needed.
  • Clinical Setting.
  • Exposure to communicable diseases, blood and other bodily fluids.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

  • Knowledge of Board of Pharmacy regulations
  • Reads, speaks, understands and writes proficiently in English.
  • Effectively communicates orally and in writing.
  • Relates and interacts with staff at all levels of the organization.
  • Represents the organization in a professional and effective manner to the community.
  • Supports, motivates and sustains a team-oriented culture.
  • Exercises sound judgment, tact and diplomacy.
  • Evaluates and effectively addresses highly complex issues and problems.
  • Delegates work, sets expectations and monitors activities of pharmacy technicians appropriately.
  • Demonstrates leadership and basic management skills.
  • Proficiency in the use of Microsoft Office applications; Word, Excel, Outlook and PowerPoint.

EDUCATION AND EXPERIENCE

  • Possession of current licensure by South Carolina State Board of Pharmacy.
  • PharmD from Accredited Institution Preferred
  • At least two years of retail or community health pharmacy experience.
