Your Rewarding Role As a Primary Care Physician (PCP), you'll be an essential member of our collaborative, results-driven team. You'll leverage your exceptional clinical skills and communication prowess to provide exceptional care to our frail elderly patients, serving as their advocate and navigator. Whether delivering in-person visits, telehealth consults, or coordinating care in skilled nursing facilities, your dedication and innovation will be instrumental in enhancing the quality of life for those in your care.

Qualifications That Make a Difference

  • Board-certified in Internal Medicine, Family Medicine, or Geriatrics, with 3+ years of primary care experience, especially caring for the elderly with complex chronic conditions
  • Adept at clinical assessment, diagnosis, care planning, implementation, documentation, and evaluation
  • Exceptional communicator and collaborative team player, focused on achieving clinical, financial, and patient satisfaction goals
  • Supportive of value-based care models and experienced in coordinated practice settings
  • Willing to travel to patient locations and participate in on-call rotations
  • Current state licensure, DEA license, BLS certification, and compliance with driving standards
