Overview:

TCC Family Health has long recognized and actively addressed the influence of social determinants of health on our patients and families. TCC is committed to eliminating barriers to care, to improve health and lower health systems costs by expanding cost-effective, meaningful and innovative health care programs that work.

The Program Supervisor reports to the Chief Health Education & Promotion Officer and is responsible for implementing, supervising and assessing current and future programs that are: 1) aimed enhancing protective factors for families with children 0-5 years of age; 2) aligned with the mission and work of the organization; and 3) support the overall strategic goals of the organization. The Program Supervisor is responsible for Coordination and implementation of assigned quality improvement activities and ensuring that all aspects of the projects/programs are completed according to applicable terms and conditions. The Program Supervisor participates in team meetings, strategic program development and provides support with writing reports. This position is also responsible for implementing reflective supervision with staff according to program guidelines in order to foster capacity building, reinforce strengths and growth within the team. The Program Supervisor effectively collaborates with TCC leadership, medical providers, staff, and volunteers to provide patient experience training/development and foster a proactive approach to improve the patient experience, organizational culture, and community relations.

Leadership Role

  • Provides oversight of Family Support Specialists (home visitors), their activities and the engagement of program participants.
  • Performs reflective supervision with Family Supports Specialists and promotes a parallel process between staff and program participants
  • Assists with emphasizing a trauma informed approach and environment that builds, supports, and promotes staff, patient and family strengths and resilience.
  • Promotes delivery of quality patient care and services
  • Supports team members in conducting timely, professional documentation in electronic health records as well as in other data tracking tools
  • Supports in tracking program outcomes as specified by grant objectives
  • Assists with effective budget/cost controls in relation to program activities and supplies
  • Assists with the completion of timely and quality reports
  • Oversees and/or leads health education classes and patient outreach activities
  • Ensures compliance with TCC policies and procedures, privacy laws and regulatory agency rules and regulations.
  • Facilitates effective project coordination between internal departments.
  • Assists with recommending personnel qualification requirements, drafting procedure manuals, and initiating in-service programs.
  • Establishes and maintains courteous, cooperative relationships with staff, volunteers and the public, portraying the mission of TCC Family Health.
  • Assists with other fundraising projects and assignments as needed.
  • Must be able to communicate effectively within the health care community and social media environments as well as with patients and their families.
  • This person must be willing to perform grassroots activities in order to meet the goals of the larger community.
  • Promotes positive working relationships with community agencies and with other departments.
  • Represents TCC at designated events and meetings.

EDUCATION:

  • Bachelor's degree in Public Health, Sociology, Child Development or related field.

EXEPERIENCE

  • Minimum of 3-5 years related post graduate experience
  • 3+ years’ experience in and understanding of effective primary prevention
  • 3+ years’ experience in early childhood (0-5) home visitation or perinatal education and supportive services

  • Must have passion for empowering under-served communities
  • Familiarity with communities in Greater Long Beach and surrounding cities, including current challenges and opportunities
  • Ability to work and thrive in a fast-paced, dynamic environment; multi-task; shift priorities as needed;
  • Healthy Families America Home Visiting model training preferred
  • Infant feeding, lactation educator or lactation consultant training preferred
  • Bilingual: English/Spanish preferred
  • Must have access to reliable transportation in order to conduct home visits
